Important information about procedures for opening a new investment

To help the government fight the funding of terrorism and money-laundering activities, federal law requires all financial institutions to obtain, verify, and record information that identifies each person who opens an account.

What this means for you: When you open a new loan or investment, we will ask for your name, address, date of birth, and other information that will allow us to identify you. We may also ask to see your driver’s license or other identifying documents.

Offering Circular

Investors are required to review our Offering Circular, which includes important information about the Community Loan Fund, and about the risk factors associated with making an Opportunity NH Investment.

Application and agreement

Our Investment Application and Agreement must be fully completed and signed. Please send the entire document to us, along with your funds, to initiate an Opportunity NH Investment. Please make check out to New Hampshire Community Loan Fund, Inc.

  • If the investment is a joint account, we require both investors' Social Security numbers and related information.
  • If the investment is for a trust, we require a copy of both the trust description page and its signature page.

W-9 (new investors only)

New investors must submit a completed and signed Tax Form W-9 with their application and funds. The W-9 form allows for only one name and social security number, or taxpayer ID number, to which the 1099-INT tax reporting will be assigned. Thank you in advance for making sure that the name and number on the W-9 are those of the primary investor listed on the Application and Agreement.

Direct Deposit Authorization Agreement

Investors who wish to receive interest payments via direct deposit must complete, sign, attach a voided check, and return this form to us.
